Video consultations are suitable for many patients as it allows the doctor to take a history to help determine a potential diagnosis and to initiate investigations that will further the patient’s care.  It is designed to be there when people are unable to come and see Dr Ricardo Jose face to face or if you need advice that does not warrant the need to visit a Doctor. Video consultations are useful when people are seeking information about any health needs. Video can be useful in diagnosing some health problems and the Dr Ricardo Jose can sometimes initiate treatment and investigations.

Video consultations can be carried out in the comfort of your own home without the need for long distance travel. This may be beneficial particualrly when a second opinion is being thought on managment of a known medical problem or you need to discuss the progress of your condition or test results with your doctor.

However, there are limitations to video consultations and it is important that you (or your carer or guardian) understand these and consent to our conditions in making this method of healthcare available to you. A major limitation of video consultations is that the doctor can’t examine you. The doctor may therefore suggest during the video consultation that a physical consultation may be needed in the clinic for you to be examined. A video consultation would not be considered appropriate if you are acutely unwell and require emergency medical care. 

You should not use video consultations if you think your health problem is urgent or an emergency. Video consultations should, under no circumstance, be used if you are experiencing a medical emergency.

If you think there is any chance that you or the person you are assisting has an urgent or emergency health need you should immediately dial 999 or seek alternative emergency medical services.

Other limitations may be technical, such as poor connectivity requiring a  consultation to be aborted. For your video consultation you should ensure that you have a good internet connection and that no one can see or overhear your consultation without your consent. 

If you have complex health needs Dr Ricardo Jose may advise that he feels that the video consultation is inappropriate for your care or that you require a physical examination. During the consultation Dr Ricardo Jose may also suggest that you should get urgent local care if this is felt necessary. 

If you are a new patient booking an initial video consultation, an electronic medical questionnaire will be emailed to you. This must be received before your consultation can go ahead. Without this singed form your video consultation appointment may be cancelled. Please bear in mind that Dr Ricardo Jose reserves the right to refuse a video consultation if he feels a video consultation will not be suitable for you. 

Follow up video consultations require all patients to update Dr Ricardo Jose on their clinical history, medicine changes, new pathology or test results and the names and contact details of new doctors involved in your care. We will endeavour to inform all doctors involved in your care if you have provided Dr Ricardo Jose with the details and unless you expressly confirm separately that you do not wish this to happen. If we do not have your GP details for correspondence, we encourage you to share a copy of our medical report with your GP so your health records are kept up to date and the GP can action any recommended plans. 

If follow up is not planned with Dr Jose, you agree to share the clinical correspondence with the clinician undertaking the follow up. 

When you consent to take part in the video consultation you agree to sharing all your relevant medical details with Dr Ricardo Jose and agree to keep Dr Ricardo Jose informed of important changes to your clinical condition.

Please submit this consent form as soon as possibe ahead of your appointment. Cancellation charges apply if the consultation is cancelled with less than 24 hours notice.
